1. No plan at all.
2. An out of date plan–kids are grown up and no longer need provisions for them when they young- Wrong persons named as Guardians for children–wrong persons named as executor or agents on power of atttorney.
3. Failure to plan for your own disability or the disability of a loved one such as a child or grandchild.
